These two disks act as isolators that protect the MOV from other components of the systems. When the MOV receives a surge current, it reacts by clamping the voltage and reducing the current to a safe level in a few nanoseconds. This surge current increases the temperature of the component, and is then sent out to the ground through those two flanges and two external leads.
